San Fernando Community Health Center Receives $275,000 in Funding from the L.A. Care Health Plan to Increase Access to Dental Care and Medical Screenings in Underserved Communities

(San Fernando, Calif. – September 8, 2023) – San Fernando Community Health Center (SFCHC) proudly announces two grants totaling $275,000 from the L.A. Care Health Plan, an organization dedicated to providing the most vulnerable residents and communities of Los Angeles County access to quality health coverage.

The L.A. Care Health Plan has selected SFCHC as one of 15 community health facilities to receive a $100,000 grant to help increase access to dental care in underserved communities. SFCHC will use the funding to provide kindergarten students important oral health screenings, as well as to educate them on the importance of oral hygiene and good nutrition. A key goal is to establish a relationship of trust among the children and the dental professionals who care for them.

“At San Fernando Community Health Center, our model of care holds the wellbeing of the whole person at its center,” said SFCHC Chief Executive Officer Audrey Simons. “A growing body of research shows a direct link between poor oral health and many chronic health conditions, such as heart disease, stroke and diabetes. This funding will help ensure children receive dental screenings, begin a lifetime of oral healthcare management and find a dental home at an early age. “

The L.A. Care Health Plan also has awarded SFCHC a Robert E. Tranquada Safety Net Initiative grant in the amount of $175,000 to provide persons dealing with health inequities access to weight management programs, as well as screenings for chronic kidney disease, high blood pressure and other chronic diseases. The funding will support the recruitment, hire and retention of culturally congruent and diverse medical staff.

“The funding will allow SFCHC to hire a registered nurse and a certified community health worker to further strengthen our primary care team,” said Simons. “The focus of our project is to increase the percentage of patients achieving blood pressure control, increase screening for chronic kidney disease and reduce obesity among patients, particularly those with underlying medical conditions. Integrating both a dedicated RN and CCHW into the team will allow better communication with patients and even higher quality care.”

(Panorama City, Calif. – August 18, 2023) – Today, Assemblywoman Luz Rivas (D-San Fernando Valley) presented a $2 million check to the San Fernando Community Health Center (SFCHC), a federally qualified health center, to open a satellite healthcare clinic. The funding for this clinic will serve the community and low-income residents of the Corazón del Valle housing project that is currently under development. SFCHC and Holos Community are collaborating on the Corazón del Valle project to ensure housing and high-quality healthcare are provided to the community members who need it most.

“The Corazón del Valle is exactly what it says it is – the heart of the valley – not only because of its location within the San Fernando Valley but because this project will change lives. Families will have a place to call home as well as receive compassionate care from healthcare providers working at the SFCHC,” stated Assemblywoman Luz Rivas. “SFCHC provides free and low-cost healthcare to over 7,000 patients in the region annually, and these state funds will increase the number of people they are able to serve. This center is one of a kind – a place where people can live and receive much-needed health care.”

“This project is a direct result of leaders in our community coming together to build a healthier, sustainable environment for those most in need,” said SFCHC Chief Executive Officer, Audrey Simons. “Corazón del Valle will enable individuals and families, who might otherwise lack access to care, to enjoy fuller and healthier lives. When we empower people with the resources to take care of their own health and wellbeing, it benefits our entire community.”

The state funding will be utilized to complete phase two of the project. The Corazón del Valle project is expected to be completed by Summer 2024.

Marking National Health Center Week, SFCHC Announces Federal Funding to Expand Critical Health Services to Children in Underserved Communities

(San Fernando, Calif. – August 10, 2023) – San Fernando Community Health Center (SFCHC), a non-profit federally qualified health center, has been awarded $200,000 to expand developmental screening for children and help ensure they have the strongest possible start in life.

Marking National Health Center Week (August 6-12), the U.S. Department of Health and Human Services has awarded $30 million to 151 Health Resources and Services Administration (HRSA)-funded health centers across the nation. SFCHC is one of 29 community clinics in California receiving a total of $5,792,165 in federal funding to improve developmental outcomes among children up to age 5 through increased screenings and follow-up services.

Federally-funded health centers are a vital source of primary and preventive care for more than 30 million people across the country, especially those living in underserved areas, regardless of their patients’ ability to pay. SFCHC will use the funding to strengthen and expand the availability of early childhood services that are vital for reducing gaps in school readiness and improving children’s ability to succeed.

Dr. Anastasia Williams, a pediatrician at SFCHC, emphasizes: "Amidst the pandemic, the family’s medical home plays a vital role in identifying developmental, behavioral and social delays in children. The pandemic’s exacerbation of these challenges makes early evaluations and referrals even more critical. By addressing these issues promptly, children are better equipped to excel in kindergarten, perform well in school and ultimately lead thriving, healthier lives. The dedicated efforts of our team focus on delivering these essential services and support to families without delay.”

According to the American Academy of Pediatrics (AAP), one in six children aged 3-17 have developmental disabilities that affect how they play, learn, speak, act or move. As they grow older, these conditions may become more severe when they go undetected. In addition, children may be at higher risk of developmental delays based on their medical history or adverse experiences, such as poverty, social inequities, abuse and neglect.

The APA recommends comprehensive screenings to identify any problem areas or missed milestones early in life. These include a developmental surveillance at every well-child visit; general developmental screenings at age 9, 18 and 30 months; autism-specific screenings at age 18 and 24 months; and referral for evaluations and early intervention services when a risk is identified.

The Biden administration has called for renewing and growing the federal investment in health centers to support critical work like meeting the developmental needs of young children. Currently, approximately 1,400 HRSA-funded health centers operate nearly 15,000 service sites in the U.S.

(San Fernando, Calif. – July 14, 2023) – San Fernando Community Health Center (SFCHC) proudly announces a grant of $50,000 from the Delta Dental Community Care Foundation, the philanthropic arm of Delta Dental of California. This funding, made possible through the Access to Care Grants Program, is dedicated to securing and increasing access to quality, affordable oral healthcare across the 15 states and Washington, D.C. where Delta Dental of California operates.

Since 2011, the Community Care Foundation has contributed more than $130 million to local community partners. The goal of these partnerships is to increase access to care, address health disparities, drive health equity and promote innovation in the oral health field.

Through this strategic grant, SFCHC can continue to provide indispensable services to the most vulnerable in the communities it serves and positively influence the lives of more individuals. As a federally qualified health center, SFCHC integrates dental medicine into primary care and behavioral health to improve patient outcomes.

San Fernando Community Health Center Launches Broadcast PSA to Help Low-Income Medi-Cal Patients Keep Their Health Coverage

(San Fernando, Calif. – May 30, 2023) – Now that the COVID-19 public health emergency is over, San Fernando Community Health Center (SFCHC) is joining a statewide effort to help low-income families keep their Medi-Cal health coverage by launching a new public service announcement (PSA) campaign titled “Stay covered and keep healthy.”

Medi-Cal is California’s Medicaid healthcare program that provides free or low-cost doctor visits, prescriptions, vaccinations, hospital visits and mental health services to children and adults with limited income and resources. For the first time in three years, the normal Medi-Cal renewal process is in effect. This means approximately 13 million people in California, or one-third of the state’s population, are at risk of losing their health coverage unless they take extra steps requalify over the next 12 months.

The 30-second radio and television PSAs, available in both English and Spanish, inform Medi-Cal patients how to keep themselves and their families covered by taking specific actions. These include updating their contact information with the county, checking their mailbox for a letter about Medi-Cal, returning their renewal form right away, and signing up for details and alerts at www.dhcs.ca.gov.

“During the pandemic, the federal government enabled people to keep continuous Medi-Cal coverage without proving eligibility,” said SFCHC CEO Audrey Simons. “Over the next year, patients will have to submit forms verifying their address, income, household size and other personal information. Many could lose their health coverage if they fail to respond or complete the required forms. As a federally qualified community health center, our mission is to empower the most vulnerable and underserved members of our community to access medical, dental, behavioral health and education services in a compassionate environment.”

(San Fernando, Calif. – March 30, 2023) – To help increase access to dental care in underserved communities, L.A. Care Health Plan is granting $1.5 million to 15 facilities that can help. SFCHC will use its share of the funding to help kindergarten students receive important oral health screenings and find a dental home.

Researchers say poor oral health is a symbol of social inequity in the U.S. People are less likely to access dental care if they are low-income, uninsured or people of color. Poor oral health can contribute to a number of diseases, and might even be a sign of chronic illnesses like heart disease and diabetes.

To help address this issue, L.A. Care launched an Oral Health Initiative to increase the number of dental professionals, enhance clinical infrastructure, and support early and long-term dental care for Medi-Cal members and other community members.

SFCHC will use up to $125,000 in funding to ensure kids receive dental screenings and begin a lifetime of oral healthcare management. This includes educating them on the importance of oral hygiene and good nutrition for strong and healthy teeth. A key goal is to establish a relationship of trust among the children and the dental professionals who care for them.

SFCHC integrates dental medicine into primary care and behavioral health to help create a healthier community.

San Fernando Community Health Center Receives Federal Grant to Host Free Vaccination Clinics Focused on Vulnerable Communities

(San Fernando, Calif. – January 31, 2023) – San Fernando Community Health Center (SFCHC) is partnering with community-based organizations to host a series of free COVID-19 and flu vaccination events funded by a $111,865 grant from the Health Resources and Services Administration (HRSA).

The walk-up clinics are part of a $350 million federal initiative for HRSA-supported health centers to increase COVID-19 and flu vaccinations in their communities, with a specific focus on underserved populations that have been hardest hit by the pandemic.

SFCHC is partnering with the City of San Fernando, MEND (Meet Each Need With Dignity) and Dignity Health to offer five free walk-up clinics in the San Fernando Valley. First, second and bivalent booster doses of the Pfizer-BioNtech and Moderna vaccines will be offered, as well as this year’s flu vaccine. Both the COVID-19 and flu shots can be taken at the same time and will be administered per Centers for Disease Control and Prevention guidelines.

“We are grateful for this opportunity to help overcome vaccine hesitancy and continue serving as a healthcare home for the most vulnerable and underserved members of our community,” said SFCHC Chief Executive Officer Audrey Simons. “As a federally qualified health center, we are uniquely positioned to offer families the care they need from an organization they know and trust. Together, with our partners, we are encouraging people of all ages to stay up-to-date with their vaccines and avoid hospitalizations, long-term health outcomes and even death.”

The first four vaccination clinics will be held at the following locations, with the final event to be announced:

(San Fernando, Calif. – July 28, 2021) – Providence Holy Cross Medical Center will award a $150,000 grant to San Fernando Community Health Center (SFCHC) to provide state-of-the-art health care services, prevention and education to patients in its San Fernando Valley service area. A check presentation ceremony and facility tour will be held on Wednesday, Aug. 4 from 11:30 a.m. to 12:30 p.m. at SFCHC’s main campus located at 732 Mott St. in San Fernando.

SFCHC, a Federally Qualified Health Center, offers patient-focused care to individuals and families regardless of income or ability to pay. The funds will be used to develop an advanced case management model that engages, navigates and links patients to primary care services in their primary language. SFCHC also will invest in expanding its care management team and developing their skills on complex population management software.

Dedicated to providing quality care to every person, particularly the most vulnerable and underserved, SFCHC operates at two locations. Its main 17,100-square-foot facility in San Fernando provides primary, dental and behavioral health care, as well as case management and health education. Its additional 1,500-square-foot facility in Mission Hills is co-located with Harbor Recuperative Care Center (HRCC) that provides housing for homeless community members recently released from local acute care hospitals.
