San Fernando Community Health Center (SFCHC) has served as a trusted, patient-centered medical home for the community since 1924. It is distinguished as a federally qualified health center (FQHC), providing comprehensive medical, dental, behavioral health and education services with an emphasis on communities that are underserved by limited access to healthcare.

SFCHC operates an approximately 17,000 square-foot main campus in San Fernando, Calif. and a satellite campus in Mission Hills, Calif. In summer 2024, It is scheduled to open a satellite clinic in Panorama City, Calif. serving low-income residents of the Corazón del Valle housing development and other community members.

Providing free and low-cost healthcare to over 7,000 patients annually, SFCHC is committed to overcoming health inequities and empowering patients to take charge if their own health. Mission

The mission of SFCHC is to provide every person quality, state-of-the-art healthcare, prevention and education services in a supportive and compassionate environment. As a FQHC, SFCHC is committed to delivering patient-focused care regardless of income, insurance status or ability to pay. Health education, literacy outreach and engagement are fundamental to achieving this mission.

History

Distinguished as the San Fernando Valley’s first community hospital, SFCHC continues its nearly century-long legacy of serving as a treasured health resource to the community. Its main campus is housed on the historic site of the original hospital, named San Fernando Community Hospital, which relocated to Panorama City in 2002 (under the dba Mission Community Hospital).

In 2004, SFCHC shifted its focus to expanding community access to diabetes prevention programs and comprehensive dental care. SFCHC opened a dental clinic in 2005, a diabetes management program called Project ALTO-D in 2006 and a Diabetes Teaching Kitchen in 2010.

In 2013, SFCHC pivoted to primary care as a non-profit, free-standing 501(c) 3 corporation. It received its California Community Clinic license in 2014 and achieved FQHC status the following year.

In 2018, SFCHC opened a satellite clinic within the footprint of Harbor Recuperative Care Center in Mission Hills that primarily serves people who are experiencing homelessness. Since its origin, SFCHC has successfully transitioned from a community hospital to a state-of-the-art FQHC that provides a full complement of medical, dental, behavioral health and education services.

From 2016-2019, SFCHC experienced an unprecedented growth rate of 284 percent. During this time, major accomplishments included expanding the primary care center and dental clinic at the main San Fernando campus, and opening a satellite clinic in Mission Hills.

In 2020, amid the COVID-19 pandemic, SFCHC launched a pilot remote patient monitoring program called Care Circle for persons experiencing difficulty in controlling diabetes. This program provides diabetes patients, particularly those from under-resourced communities, with tools that empower them to self-manage their care, overcome health inequities and improve outcomes while living with a chronic disease.

SFCHC continuously links its primary care services to major health plans and healthcare institutions to better provide individualized and specialized care. It offers employment opportunities within the local community to the fullest extent possible, creating jobs and fueling the economy.

Recent Accomplishments

In summer 2023, SFCHC announced construction of a satellite health clinic within the footprint of the Corazón del Valle housing project that is will open in 2024. Thanks to $2 million in state funding secured by Assemblywoman Luz Rivas, this beautifully designed Holos Communities development will help increase health equity, assist the unhoused and strengthen our community.

In fall 2023, SFCHC was selected to by Providence-Holy Cross Medical Center as a partner in the Los Angeles County Department of Public Health’s Community Public Health Teams (CPHT) pilot project. The goal of this program is to build a community-centered system of care that delivers coordinated health and social services to those most in need.

In fall 2023, SFCHC launched a new Mobile Medicine Program bringing critical medical and behavioral health services directly to unhoused individuals and families in the San Fernando Valley. Funded by a $1 million grant from Providence Health Services, the program extends the delivery of healthcare services to individuals and families who are living with the complexities of homelessness.
