Providence Holy Cross Medical Center Awards $150,000 Grant to San Fernando Community Health Center to Benefit Underserved Communities in the San Fernando Valley

(San Fernando, Calif. – July 28, 2021) – Providence Holy Cross Medical Center will award a $150,000 grant to San Fernando Community Health Center (SFCHC) to provide state-of-the-art health care services, prevention and education to patients in its San Fernando Valley service area. A check presentation ceremony and facility tour will be held on Wednesday, Aug. 4 from 11:30 a.m. to 12:30 p.m. at SFCHC’s main campus located at 732 Mott St. in San Fernando.

SFCHC, a Federally Qualified Health Center, offers patient-focused care to individuals and families regardless of income or ability to pay. The funds will be used to develop an advanced case management model that engages, navigates and links patients to primary care services in their primary language. SFCHC also will invest in expanding its care management team and developing their skills on complex population management software.

“We are grateful for this opportunity to continue our legacy of serving as a trusted, patient-centered medical home for our community since 1924,” said SFCHC Chief Executive Officer Audrey Simons. “Our ultimate goal is to establish a trusting and informative relationship with clients, and welcome them to a compassionate medical home that meets their individual needs. Our experience working with the most vulnerable members of our community teaches that communicating in a patient’s primary language, culture and at their level of health literacy empowers them to make the best medical choices.”

Event participants will include SFCHC Chief Executive Officer Audrey Simons; SFCHC Chief Operations Officer Heidi Lennartz; Providence Holy Cross Medical Center Chief Executive Bernard Klein, M.D. and Providence Director, Community Health for San Fernando Valley Anthony Ortiz Luis. Excellent photo and interview opportunities will be available.  All COVID-19 health and safety protocols, including masking, will be observed.
