POSITION SUMMARY:
The Health/Nutrition Educator promotes health through nutrition by designing, implementing, and teaching nutrition techniques and DSME programs. The Health/Nutrition Educator reports to the Health Education Manager.

CORE JOB RESPONSIBILITIES:

• Develop and administer curriculum for the Diabetes Teaching Kitchen Program.

• Use pre‐approved curriculum and prepare presentations to promote healthy eating and nutritional information.

• Coordinate weekly nutrition classes which include but are not limited to food purchasing and food preparation for scheduled program classes.

• Conduct outreach with non‐profits and educational institutions for participants and/or possible locations for off-site classes within grant funded focus areas.

• Coordinate reporting and recordkeeping requirements for those served in the Diabetes Teaching Kitchen Program.

• Maintain food safety compliance standards as required by the Department of Public Health.

• Order appropriate equipment and materials needed to conduct the Diabetes Teaching Kitchen Program.

• Engage and leverage local partnerships to assist the Diabetes Teaching Kitchen Program.

• Maintain current relationships with California State University Northridge to serve as a preceptor site for dietitian interns to work with the Diabetes Teaching Kitchen Program.

• Develop and administer curriculum for DSME Project ALTO-D and DSMT program

• Document the nutrition diagnostic statement in PES (problem, etiology, and signs/symptoms) and provide supporting evidence within the progress note.

SKILLS/QUALIFICATIONS:

• Presentation skills and nutrition education experience.

• Health promotion and maintenance.

• Teaching experience.

• Tracking budget expenses and conducting food research

• Excellent written communication and computer skills

• Time management skills

• Ability to work with others and work independently

• Knowledge of nutrition, health education, care process, and an ability to apply all steps.

Required License/Certificates:

• Bachelor's degree (B. A.) from an accredited four-year college or university in the field of nutrition, health, or a related academic field.

• An equivalent combination of training and experience may be substituted for the educational requirement. 

  California Driver’s License:  Required for this classification.

Registered Dietitian

POSITION TITLE:

POSITION SUMMARY:
This position exists to provide nutrition consultation and education to patients and their families who are screened as nutrition risk, identified with nutrition risk, and/or referred by physicians or other healthcare providers for nutrition-related problems, chronic disease management, management of nutrition support, to meet regulatory requirements, or for prevention and wellness. The RDN is an integral part of the multidisciplinary team who serves as the patient’s nutrition advocate. The RDN ensures that the patient’s nutrition needs are being met, including when nutrition support (enteral or parenteral) initiation and/or management is necessary, during patient care, clinic visits, and unit/clinic rounds. This position provides the incumbent with a hybrid schedule of on-site and remote attendance sessions, with remote days to be worked out with the Health Education Manager.

CORE JOB RESPONSIBILITIES:

• Conducts nutrition assessment when warranted by patient’s needs or conditions or warranted by a regulatory requirement.

• Selects assessment tools and procedures (matching the assessment method to the situation) and applies assessment tools in valid and reliable ways.

• Reviews dietary intake for factors that affect health conditions and nutritional risk.

• Utilizes medical history, laboratory data, drug profile, anthropometric measurements, and diet history to evaluate for nutrition-related consequences.

• Performs nutrition-focused physical exam (NFPE) during assessments on patients to determine risk and/or degree of malnutrition when indicated.

• Evaluates psychosocial, functional, and behavioral factors related to food access, selection, preparation; physical activity; and understanding of health condition.

• Evaluates patients' and family’s knowledge, readiness to learn, and potential for changing behaviors.

• Observes patients and family for nonverbal and verbal cues that can guide and prompt effective interviewing methods.

• Organizes and categorizes the data in a meaningful framework that is related to nutrition problems.

• Determines when a problem requires consultation or referral to another provider or service.

• Finds patterns and relationships among the data and possible causes for the nutrition problem.

• Prioritizes the relative importance of nutrition problems for optimal patient health, recovery from illness, management of chronic disease, or disease prevention.

• Documents the nutrition diagnostic statement in PES (problem, etiology, and signs/symptoms) and provides supporting evidence within the progress note.

• Formulates and determines a plan of action that is focused on the etiology of the problem and known to be effective based on best current knowledge of evidence from Evidence-Based Nutrition Practice Guidelines.

• Coordinates care with colleagues to ensure appropriate transitions of care are addressed to meet patient’s needs.

• Acts as a resource for medical/clinical staff on the nutritional management of patients.

• Interprets and presents current clinical knowledge, standard of care, and research to improve patient care and the nutrition service line.

SUPPORTING JOB RESPONSIBILITIES

• Attend workshops, training, and meetings as needed, and as requested.

• Perform other duties as assigned by the Health Education Manager/Director of Medical Service.

QUALIFICATIONS:

Required education, experience, training

• Bachelor’s degree in Dietetics and Nutrition, Health Education or Health Care Management with a focus on Nutrition or Dietetics.

• At least two recent years of experience with managing patients in a health clinic.

• Must have experience in building highly productive and collaborative relationships with multi-disciplinary teams.

• CDR Registered Dietician

• Knowledge of nutrition care process and ability to apply all steps.

• Excellent communication and presentation skills

Preferred education, experience, training

• Master's Degree in Dietetics and Nutrition, Health Education, or a related field with emphasis on nutrition and health education.

• Working knowledge of medical nutrition therapy, human behavior in a multi-ethnic population, techniques for effecting behavior change.

• Experience working in e-Clinical Works.

POSITION SUMMARY:
This physician specialist is responsible for overseeing and managing the care of patients assigned. The Physician specialist actively participates in Quality/PI, Peer Review and Patient Safety programs as directed by the CMO.  In addition, the Physician specialist also promotes the integration of clinical programs and ensures medical care is consistent with the mission and vision of San Fernando Community Health Center (SFCHC). The family practice/Internal medicine physician specialist complies with all regulatory requirements for the provision of medical and behavioral health services. The Physician specialist reports directly to the Chief Medical Officer of SFCHC. 

CORE JOB RESPONSIBILITIES:

• Provides leadership and direction in the initiation and continuation of primary care services at SFCHC.

• Examines individuals requesting primary care services.  Provides accurate diagnosis and appropriate care within the scope of Internal medicine and refers individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines.

• Documents all patient interactions in EHR to accurately reflect the nature of the encounter, the condition of the patient and the care or treatment provided. 

• Works collaboratively with CMO with integration of clinical areas within SFCHC including but not limited to medical services, dental services, behavioral health services, and other services as identified from time to time.

• Ensures compliance with federal, State and local regulation associated with providing medical services at SFCHC Clinics.

• Works collaboratively across multidisciplinary lines to improve the provision of care.

• Works collaboratively with Administration to ensure effective and efficient clinical operations.

• Assists CMO with the provision of the training of the support staff.

• Supervises mid-level providers.

• Participates in Peer Review activities.

• Reviews and collaborates on all practice protocols for providers and clinic policies and procedures related to the provision of care to patients.

• Maintains patient confidentiality in accordance with HIPAA regulations.

• Assists with the maintenance of clinical coding and documentation standards.

• Complies fully with all applicable federal, state, local and center rules, regulations, protocols and procedures governing the practice of medicine as well as those relating to, but not limited to, personnel issues, workplace safety public health and confidentiality.

QUALIFICATIONS:

Required education, experience, training

• Must be board certified in Family Medicine (AOBFP or ABFM0 or Internal Medicine (ABIM or AOBIM);

• Recognized M. D. degree;

• Must have completed a residency approved by an American Specialty Board of Internal Medicine/FP; and,

• Credentialing approved by SFCHC Board of Directors

Additional experience, licensing, training

• Licensed by the State Board of Medical Examiners to practice medicine in the state of California.
