POSITION SUMMARY:
The Director of Care Management oversees the implementation, development, and management of the care management programs. This position ensures compliance with all regulatory requirements, establishes efficient processes for patient care, and provides leadership to the Care Management team. The Director collaborates with case management, behavioral health, and medical providers, and community resources to deliver patient-centered care and achieve optimal outcomes. The incumbent manages and supervises onsite case managers and reports to the Chief Medical Officer, with additional reporting responsibilities to the facility leadership. The ideal candidate has experience in management, working at a facility as part of a treatment team, and in business development.
CORE JOB RESPONSIBILITIES:
Develop and communicate continuum of care processes across the clinic
Oversee day-to-day care management program operations, ensuring efficient and effective delivery of services
Drive organizational commitment, executive buy-in, use adoption, execution oversight, and key metrics for implementation of key continuum of care strategies, investments and initiatives
Supervise and provide clinical and administrative oversight to all staff involved in care management activities, fostering a supportive and high-performance work environment.
Establish an annual zero-based budget that aligns to SFCHC’s continuum of care philosophy and strategic plan and meets or exceeds budgeted operating margin for SFCHC
Establish relationships and linkages with key clinical, finance, and ancillary support teams across the organization
Establish key performance indicators and related financial reports to gauge continuum of care performance against plan.
Establish effective continuum of care governance framework across the organization
Maintain and communicate prioritized portfolio of projects aligned to continuum of care strategy
Support and lead implementation of all continuum of care-related initiatives
Work collaboratively and build relationships with clinic leaders, C-suite executive team, and others to ensure excellence in the delivery of an integrated services model (HIV, infectious disease, behavioral health, and health education) that comprehensively and seamlessly addresses patient needs.
Strengthen provider relationships, build trust, and gain provider engagement for enhanced adoption of continuum of care clinical and financial systems as it relates to integrated patient quality management
Provide leadership to execute on system quality, operational and capital budgets, consistent with agreed on timelines and objectives
Develop and operationalize care policy and processes in conjunction with CMO and Medical Clinic Director
Improve case management and social workers’ competencies to achieve proficiency on job-specific competency tests
Measure and consistently improve provider satisfaction metric in annual agreed on increments
Create and keep updated continuum of care policy and procedure manual, including managing its implementation
Develop and implement clinic-wide continuum of care strategy
SUPPORTING JOB RESPONSIBILITIES:
Attend workshops, training, and meetings as needed, and as requested.
Perform other duties as assigned by COO or Administration.

QUALIFICATIONS:
Required education, experience, training
Master’s degree in a healthcare-related program of study with nursing or social work preferred;
Minimum of five years’ experience in directing a continuum of care/integrated case management department with a minimum of ten years in an FQHC or similar clinic; related experience in a hospital may be considered
Must be a Licensed Clinical Social Worker
Appropriate licenses as required
Additional experience, training
Successful experience as part of a productive and collaborative healthcare team, networking, driving change and improvements within multi-disciplinary teams.
Working knowledge of integrated behavioral health model within a primary care setting.
Applicable experience with e-Clinical Works EHR system and other interoperable software.
Experience with developing and delivering staff training and in-services preferred.
The ideal candidate has experience in management, working at a facility as part of a treatment team, and in business development.

$140,000 to $160,000.00
