San Fernando Community Health Center has served as a trusted, patient-centered medical home for the community since 1924, providing comprehensive medical, dental, behavioral health and education services with an emphasis on groups underserved by limited access to healthcare.

What’s the History of SFCHC?

San Fernando Community Health Center (SFCHC) has a nearly century-long legacy of serving as a treasured health resource to the community. Distinguished as the San Fernando Valley’s first community hospital, SFCHC operates an approximately 17,000 square-foot main campus on the historic site of the original San Fernando Community Hospital. As we’ve grown, we’ve honored that hospital’s legacy:

2001
San Fernando Community Hospital closes

2004
SFCHC shifts its focus to community health care through programs for diabetes prevention and dental care.

2005
SFCHC opens a dental clinic

2006
SFCHC begins a diabetes management program called Project ALTO-D

2010
SFCHC opens its Diabetes Teaching Kitchen

2014
SFCHC becomes a Federally Qualified Health Center!

2018
On the former site of Harbor Recuperative Care Center, SFCHC opens a satellite clinic in Mission Hills that primarily serves people experiencing homelessness.

Summer 2024
SFCHC will open a satellite clinic in Panorama City, serving low-income residents of the Corazón del Valle housing development and other community members.

What Programs Can SFCHC Offer You?

SFCHC is proud of the primary care and specialty services that our excellent team of providers, clinical caregivers, and support teams deliver.

  • The Medical, Dental, Behavioral Health, and Case Management teams coordinate a full range of services, including prevention education and services for diabetes, hypertension, and HIV.

  • SFCHC provides first-class OB, gynecologic, and pediatric care for mothers and children from infants to adolescents, including the outstanding California Perinatal Services Program.

  • Our Dental Team is an exceptional combination of professionals and dedicated caregivers who understand the importance of oral health in overall health and wellness.

  • Our Health and Wellness Education Team has improved patient outcomes through our Care Circle Diabetes Program, which includes cooking classes in our premier Diabetes Teaching Kitchen.
